Anthony Martinez

With over 30 years of hospice leadership, my mission is to serve patients and families at the highest level possible. My personal philosophy, “Success Through Servanthood,” is lived out daily by both myself and our team. I believe that you cannot give what you do not have; I promise that our team will care for you as we would our own family members.

Having personally experienced the hospice journey, I am committed to serving you and your family with genuine compassion, empathy, and love. We look forward to serving you.

Jacki Turner

A proud Houston native, I have spent my life rooted in this city and nearly two decades dedicated to serving others through healthcare. With a professional background in social work, my career has always been centered on advocacy, compassion, and helping individuals and families navigate some of life’s most important moments with dignity and support.

As the Administrator and Owner of Your Choice Hospice and Palliative, I am deeply committed to building something truly meaningful—an organization grounded in compassion, trust, and personalized care. At Your Choice, our vision is to provide families with thoughtful guidance, exceptional support, and the reassurance that their loved ones are cared for with dignity, respect, and heart.

Outside of work, I am a wife, mother of six, and proud grandmother of two. Family is at the center of everything I do and continues to shape the way I lead and serve others. In my personal time, I enjoy rescuing dogs, spending time in nature, and traveling whenever I can.

Raquel Sonali Vela

A healthcare professional with over two decades of experience, including more than ten years in sales and business development. She is passionate about expanding access to compassionate, patient-centered hospice and palliative care while supporting families through life’s most meaningful moments.

She has held roles such as Territory Manager, Business Development Director, and Regional Expansion Manager, helping develop programs and build strong relationships within the healthcare community.

As part of the leadership and ownership team at Your Choice Hospice and Palliative Care, Raquel is committed to growing an organization rooted in compassion, integrity, and excellence. She works closely with providers and families to ensure every patient receives high-quality, thoughtful care. She is also bilingual in English and Spanish.

Outside of work, Raquel enjoys spending time with her husband, family, and friends, along with her dog, Toby, and enjoys nature, creative hobbies, and reading.

Dr. Anna Cecilia Tenorio

Dr. Anna Cecilia Tenorio, MD is a board-certified physician in Family Medicine and Hospice & Palliative Care, and the founder of AppleMD Medical Services. She is committed to delivering patient-centered care that prioritizes comfort, dignity, and quality of life for those living with serious illness.

With a deep dedication to patients facing complex medical conditions, Dr. Tenorio takes a thoughtful and individualized approach to care—listening closely, aligning treatment with each patient’s values, and supporting families through challenging decisions. She is especially known for her compassionate presence in end-of-life care, providing guidance, comfort, and clarity during some of life’s most difficult moments.

Her goal is to ensure that every patient feels truly seen, heard, and cared for with empathy, respect, and unwavering support.

Leo Guevara

Born and raised in Houston, Texas, Leo discovered his passion for healthcare and helping others at an early age. He attended Michael E. DeBakey High School for Health Professions, earned his Certified Nurse Assistant certification, and gained hands-on experience as a nurse assistant while still in high school.

Leo later served in the U.S. Navy as a hospital corpsman attached to a Marine unit, working in labor and delivery before earning his nursing degree in California. After returning home to Houston, he found his calling in hospice care and went on to earn his bachelor’s degree in nursing from the University of Houston-Downtown in 2021.

With more than 13 years of experience, Leo is known for his compassion, integrity, and dedication to supporting patients and families through life’s most meaningful moments. As owner, he brings both heart and leadership to his work and remains deeply committed to serving the Houston community.

Outside of work, Leo enjoys spending time with his wife and two daughters, traveling to visit family, swimming, attending sporting events, and cheering on the Houston Astros.
